Retail Meat Pension Fund
Note: The Independent Packing Houses (IPH) Pension Fund has merged into the United Food and Commercial Workers Union Local 152 Retail Meat Pension Fund
This Plan was established and is maintained under collective bargaining agreements between your Employer and Local 152, or participation agreements between an Employer and the Plan. These agreements call for Participating Employers to make contributions to the Plan. Employees do not contribute to the Plan. You become a participant when your Employer is first obligated to make contributions in respect to your employment.
PENSION BENEFIT STATUS REQUEST
To request a Pension Benefit Status (an estimate of your projected benefits based upon currently available information), please download and print the form and mail it to the Fund office.
PLANNING TO RETIRE
If you are planning to retire, please contact the Fund office six (6) months prior to your anticipated retirement date and we will send you the Pension Benefits Application Package, which includes a Pension Application, Benefit Election Form, Certificate of Marital Status Form, Spousal Waiver Form, and an Affidavit.
Pension benefits are effective as early as the first of the month following termination from covered employment and receipt of a complete pension application.
DIRECT DEPOSIT (for retirees collecting pension benefits only)
Direct Deposit is a simple, safe and secure way to receive your monthly pension benefits check into a bank or credit union account.

If you would like to have your pension payments deposited directly to your bank, please complete the Automatic Direct Deposit Form. Please be sure the following information is completed.
• Name of financial institution (bank or credit union)
• Routing transit number for your bank or credit union
• Account type (checking or savings)
• Account Number
You can also contact the bank or credit union to help you complete the direct deposit form.
TAX WITHHOLDING
You have the right to change or revoke any election made by you to have or not to have Federal Income Tax withheld from your pension check. To change or revoke your election, please complete the Federal Income Tax Withholding Election Form and mail it to the Fund Office. This form is provided for your convenience purposes only.
